IMPORTANT: Once you purchase a Health Center membership, if you do not already have key cards, please email head shot pictures of each family member listed on the membership (16 years or older) to OCC@oakwoodohio.gov and we will assign permanent key tags for each of you that you can pick up at the OCC front desk. Please be sure to label the pictures so we can identify each family member by name. These key tags are your permanent tags and will be used with all Memberships you purchase and for future Membership purchases as well. If you misplace your key card, we do sell replacement ones for $5.00 each. You must have a key tag to gain entry to the Health Center.
Only people who reside PERMANENTLY at this address may be included in this membership.
Health Center Membership Fees are non-refundable/non-transferable.
Health Center Membership
The membership entitles members ages 16 and older full use of the Fitness Room during posted hours. Signed parental permission is needed for 16 and 17 year olds. You will be asked to sign a Release and Indemnity Agreement before your first visit and every year thereafter.
When purchasing a membership you will be asked for a date. If this is a new membership please select the date of purchase or if you are renewing your membership(s), the start date would be the day after your current membership expires. Membership is valid for one (1) year from the time of purchase.
